Initial Assessment Details
60-90 minute session includes:
Full assessment and diagnosis, (including referral for imaging if required)

If you have private health insurance with Physiotherapy extra cover, please bring your card for on the spot claiming.
As everyone has a different level of cover, please contact your health insurance provider to confirm your exact rebate (Initial Physiotherapy Code 500)

How To Find Us
We're located at 2/385 Fullarton Road, Fullarton. This is the western side of Fullarton road, between Wattle and Fisher streets.
We share the space with Equilibrium 4 Health (look for red sign out front).

Prior to your appointment
Please arrive 5-10 minutes early to fill out some paperwork.
You can also download the forms below and, if possible, email them to dan@kinfolkwellness.com.au at least 24 hours prior to your appointment. If you have a copy of any reports, please also include them in the email.
Greet
Dan your Physiotherapist will then greet you in the waiting room and take you through to the treatment area. Your assessment and treatment will be in a private treatment room. We consider your privacy to be of paramount importance.
All conversations and records are kept confidential. We will only release information about you and your condition to other health practitioners or specialists with your approval.
We Listen
We listen to your story and learn about your values and what matters to you. We work together to define your short and long terms goals for treatment.
After a full health history check, we will ask some questions to find out more about your current and past injuries.
If you have any reports from previous scans (x-rays, MRI, ultrasounds e.t.c) please email them to dan@kinfolkwellness.com.au
We then guide you through an extensive evaluation to test your body's movement and function.
Feel Better
Using a combination of education and innovative self-manual therapy techniques, we aim to reduce your pain and get you feeling better, right from Day 1.
We use an evidence-based approach that looks at the total body connection rather than isolated muscles or joints.
Move Better
Towards the end of the session, we'll teach you some exercises so that you maintain the gains you've made during the session.
After your session, Dan will spend time building a customized and detailed plan to re-build your foundation and get you feeling ‘better than ever before’.
You will receive a written summary of your diagnosis, goals and 'plan of attack' with an idea what we can do to help you and what you can do to help yourself.
What To Wear
Please wear loose fitting comfortable clothing so that the injured part can be examined.
Towels will be provided to ensure you are comfortable throughout the treatment. If you're coming from work, you can always bring a change of clothes and change while the physio steps out.
Be prepared to move!
Also please have your phone ready because we'll be creating a personalised movement plan to address the root cause of your issues.

What To Send Us

Payment
Payment is required at the time of treatment. Payment can be made with cash, debit card or credit card (Visa or MasterCard).
Depending on your private health insurance arrangements, we have an HICAPS facility, which lets you claim on the spot and then only need to cover the gap.
Please bring your private health insurance card to take advantage of HICAPS.
